For instructions on how to get a prescription filled for your Wiley X model, please read the following; then download either Prescription Order Form: XLS or PDF
Ordering “Lenses Only” or “Rx Insert Only”
If you need Rx lenses only for your SG-1 please select the “SG-1K Lens Cylinders Only” option and write in how many pair that you need in the quantity field. If you need the "Rx Insert Only” for the: CQC, NERVE, SPEAR, PT-1 or PT-3 please select the corresponding "Insert Only" option and write in how many pair that you need in the quantity field. Please note that you will not be eligible for the $5.00 discount from the total cost if you do not send in your glasses with the order. If paying by credit card, please fax order form with complete prescription to # 925-215-2447. If mailing check, mail it with order form and a copy of your complete prescription to the address below.
Sending in your own glasses (Includes XL-1, ROMER II or any model from Active, Street, or Climate Control Series)
If sending in your own glasses to be filled with your prescription, please complete the order form of your choice(see links above) and mail it in with your glasses and copy of your prescription. To guarantee receipt on our end, please use only a shipper that provides package tracking (i.e. U.P.S., FedEx, DHL, etc.) We are not responsible for lost packages sent by U.S. Mail. Be sure to deduct the $5.00 discount from your total cost when choosing this option. Please Note: the XL-1 and ROMER II models will no longer be interchangeable with prescription lenses.
Mail in your form, a copy of your prescription, and your glasses to Wiley X, Inc. at
