For instructions on how to get a prescription filled for the SG-1, XL-1, CQC, NERVE, SPEAR, PT-1, PT-3 or Romer II models, please read and choose one of the following options; then download the Prescription Order Form: PDF or MS Word

Ordering Lenses Only for the: SG-1, CQC, NERVE, SPEAR, PT-1 or PT-3
If you need Rx lenses only for your SG-1 please select the “SG-1K Lens Cylinders Only” option and write in how many pair that you need in the quantity field. If you need the Rx Insert for the: CQC, NERVE, SPEAR, PT-1 or PT-3 please select the corresponding "Insert Only" option and write in how many pair that you need in the quantity field. Please note that you will not be eligible for the $5.00 discount from the total cost if you do not send in your glasses with the order. If paying by credit card, please fax order form with complete prescription to # 925-455-8860. If mailing check, mail it with order form and a copy of your complete prescription to the address below.

Sending in your own glasses: XL-1 & Romer II
If you are sending in your glasses to be filled, fill in the order form and mail it into us along with your glasses and a copy of your prescription. To guarantee receipt on our end, please use only a shipper that provides package tracking (i.e. U.P.S., Fedex, DHL, etc.) We are not responsible for lost packages sent by U.S. Mail. Be sure to deduct the $5.00 discount from your total cost when choosing this option. Please Note: the Romer II and XL-1 will no longer be interchangeable with prescription lenses.
